Indications

This test is performed to check if the adrenal glands are producing the right amount of the hormone cortisol

Comments/Precautions

Please refer to local trust guidelines for DFT protocols Adrenal failure. (NB Prednisolone cross reacts with cortisol assays. This information is for use with adults not children). No dietary restrictions for this test. Test should be performed in the morning when cortisol levels are at highest. Please ensure all timings are clearly labelled on every sample.

Sample type

Refer to local trust DFT protocol. General guidance: • Take one serum (yellow top) sample of blood for cortisol clearly marking the bottle as '0' minutes. • Inject Synacthen intramuscular (check protocol for dose) • 30 mins after injection take further serum (yellow top) sample of blood. Clearly mark tube '30' minutes. • 60 mins after injection take further serum (yellow top) sample of blood. Clearly mark tube '60' minutes.

Reference intervals (adult unless stated)

30 min Cortisol should be above 450 nmol/L
